What Is Histrionic Personality Disorder (HPD)?

HPD is characterized by unstable emotions, a distorted self-image, and an overwhelming desire to be noticed. This often leads to dramatic or inappropriate behaviors to gain attention. People with HPD are frequently described as seductive, self-indulgent, dramatic, extroverted, and animated.

Who Is Affected by Histrionic Personality Disorder?

Women are more commonly diagnosed with HPD than men. However, researchers now believe that may be the result of underdiagnosis.

In most cases, symptoms and characteristics of histrionic personality disorder do not fully develop until late adolescence or early adulthood, while treatment often begins around age 40.

Signs and Symptoms of Histrionic Personality Disorder

Signs of HPD may include:

  • Being dramatic and emotionally expressive.
  • Being overly concerned with their physical appearance.
  • Being persistently charming and flirtatious.
  • Being inappropriately sexual with everyone, regardless of familiarity.
  • Thinking that relationships are closer than they really are.
  • Having difficulty maintaining relationships.
  • Needing instant gratification, approval, and reassurance.
  • Having rapidly shifting and shallow emotions.

What Causes Histrionic Personality Disorder?

Histrionic personality disorder is one of the least understood mental health disorders. While its exact cause remains unknown, several factors may influence its development.

Genetic Influences

Histrionic personality disorder often runs in families, which suggests there is a genetic link.

Childhood Trauma

Child abuse, neglect, or the loss of a loved one may contribute to histrionic personality disorder later in life.

Parenting Styles

The following factors can negatively impact a child’s development, leading to low self-esteem, emotional dysregulation, relationship difficulties, and personality disorders.

  • Lack of boundaries, overindulgence, or inconsistent attention.
  • Model dramatic, erratic, volatile, or sexually inappropriate behavior.
  • Lack of clear expectations.

Diagnosis of Histrionic Personality Disorder

According to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5), a person must have five or more of the following persistent behaviors:

  • Needs to be the center of attention.
  • Seductive or provocative behavior.
  • Shifting and shallow emotions.
  • Uses appearance to draw attention.
  • Impressionistic and vague speech.
  • Dramatic or exaggerated emotions.
  • Suggestible (easily influenced by others).
  • Thinks relationships are closer than they are.

Treatment for Histrionic Personality Disorder

Psychotherapy, also called talk therapy, is the preferred treatment for histrionic and other personality disorders at Beach House Center for Recovery. This approach helps you identify and address the motivations and fears associated with your thoughts and behaviors.

From there, you can learn to relate to others more positively, naturally, and healthily.

Psychodynamic Therapy

This approach uses self-reflection and self-examination to help you better understand problematic relationship patterns. It also encourages you to develop awareness of your emotions and process unresolved feelings. Three common techniques include:

  • Free Association
Self-directed exploration, guided by a licensed therapist to identify thought patterns and reflect on your feelings.
  • Projective Tests
Word association or inkblot testing to identify your subconscious thoughts and feelings.
  • Dream Analysis

A subjective technique to help understand hidden meanings and fears and identify areas for growth.

Supportive Therapy

Supportive therapy for histrionic personality disorder focuses on empathy and active listening. It helps:

  • Manage symptoms.
  • Build secure attachments.
  • Improve self-esteem.
  • Establish effective coping skills.

This evidence-based approach can be used in individual, group, or family therapy sessions as it helps decrease emotional distress and improve overall well-being.

Cognitive Behavioral Therapy (CBT)

CBT is a structured and goal-oriented therapy guided by a licensed therapist. It can help people with HPD unwind negative behaviors and thoughts and learn healthier thinking patterns and habits.

Group Therapy

Group therapy can help you practice new social skills, understand why you behave the way you do, and develop healthier ways to interact with others in a safe, supportive, and nonjudgemental environment. However, due to the attention-seeking nature of HPD, group therapy is often used later in the treatment process.
