What are the additional expenses that I am expected to be responsible for?
You may be responsible for a portion of the admission fee, a deductible, a coinsurance percentage and/or a copayment, depending on whether you have an HMO, PPO or EPO plan. For example, with the Direct HMO plan, you are eligible for 60 outpatient visits, including 20 family counseling visits per calendar year, at no cost to you. With the Empire Total Blue Choice Plan, the same services require your contribution of a deductible and 10% coinsurance. Consult your policy for details.
